In order to apply please submit the Application for Admissions to the front desk at Promise Academy.

You can also upload your application using the form at the bottom of this page. First, download the file to your computer. Once you have completed the application be sure to save the student information to the application and change the file name to include the applicant’s name. Fill in the form below, drag and drop the completed application, and click submit.

Once your information is received you will be contacted to schedule a school tour and student assessment. Please bring the following documentation to your scheduled meeting.

  • Birth certificate
  • Immunization records
  • Most recent report card (2nd-5th)
  • Previous years W2 (To establish Fair Share Tuition)

Applicants must be at least five years old by September 1. If your child is not five by September 1, your application will not be considered. There are no exceptions.

Student Assessments 

  • Kindergarten through Grade 1 – Observation for school readiness
  • Grades 2-5  – Testing by appointment on an individual basis

After official acceptance parents must submit $30 non-refundable deposit to hold your child’s spot.
